CGHA Player Release Policy & Request for Release
A player Release or Permission to Skate is not required for movement to and from boys’ hockey associations as they are not part of the OWHA. However, if the player was with another girls association prior to playing in a boys association, then she will need to obtain a Release from that girls association before playing with CGHA.
A player will also be required to produce a Release from her most recent association even if she has not played for a number of years. The OWHA player release form can be found at the link below.

Permission to Skate form (PTS)
As of September 2024, it is the CGHA's decision that "Permission to Skate" with another association will no longer be issues for players wishing to try out for another association. If a player wishes to tryout for another association, they will be granted a release.
Request for Release
Players wishing to tryout for another association will be required to request a release from the CGHA. This will commence as of April 1, 2024 in an effort to reduce the volume of requests overseen by the executive.
The request for Release will be submitted to the registrar. Please note, up to 7 days will be required to secure signed releases upon proper notification. The player release form can be found at the link below and submitted to tammyjette1@gmail.com.
Once released, a player may submit an application to return to the CGHA but permission to return will be dependent on space/availability and executive approval.

CGHA Declaration of Conviction
This Declaration of Conviction form is to be completed by each Team Officials each year for 3 years immediately following the last Criminal Background Check.

Players, Parents & Team Officials Fair Play Pledge
This form must be signed by the Players, Parents & Team Officials and submitted to their coach and/or manager at the beginning of the season. It is the intention of this pledge to promote fair play and respect for all participants within the Cornwall Girls Hockey Association. All players, parents & team officials’ must sign this pledge before being allowed to participate in hockey and must continue to observe the principles of Fair Play.
